We’re excited to support you here at UWEC!

To request accommodations, please start by filling out a short application. This helps us learn more about you and your needs.

Documentation will be needed in addition to the application. There is a section within the application where it can be attached if it is in electronic format.

Please note: If you are working to get documentation, we still encourage you to submit the application for accommodations. Documentation can still be provided to our office after submitting the application. Materials will be reviewed after an application and documentation are on file.

Looking for Accommodations for the 鶹Ƶ Placement Tests? Those can be requested by . The SSD Office at UWEC provides accommodations for students while they are on campus. Please contact the Academic Testing office by email at academictesting@uwec.edu or by phone at 715-836-5522 with any questions.

Provide Supportive Documentation

Disability Documentation Requirements

To ensure the provision of reasonable and appropriate accommodations on the basis of a disability, students requesting accommodations must provide documentation of their disability as defined by federal law. Title II of the Americans with Disabilities Act (ADA) of 1990 as amended and Section 504 of the Rehabilitation Act of 1973 define a disability as a physical or mental impairment that substantially limits one or more major life activities.

Disability documentation should include:

  • a clear diagnostic statement
  • information on the severity of the condition and the resulting impact on a major life activity
  • details of the typical progression or prognosis of the condition.

In addition, eligibility for academic accommodations is based on the following:

  • data in the documentation that clearly demonstrates that a student has one or more functional limitations within an academic setting
  • these functional limitations require accommodation in order to achieve equal access.

Documentation is not required at the time of submitting your online application for accommodations. It can be provided after submitting an application. Materials will be reviewed after an application and documentation are on file.

Our Comprehensive Disability Documentation form, which can be found below, is a helpful resource for you and your provider for what our Accommodations Coordinators look for in documentation. Please note that this form is optional to fill out. If you have other forms of documentation that follow the guidelines above, those can be provided as well. 504 Plans and IEPs are encouraged to be provided to our office.

In addition to uploading documentation to the online application, it can be emailed to us at ssd@uwec.edu, faxed to 715-318-5559, or dropped off in our main office.

If you are a student attending UW-Barron County campus, please submit disability documentation to the UW-Barron County contact.

Once everything is provided to us, our Accommodations Coordinators will email with the next steps. Which could be asking for more documentation or to set up an Intake Meeting where they will discuss specific accommodations and help coordinate those.

All communication will be sent to the UWEC email account, so please make sure it is activated.

Requesting Copies of Disability Documentation 

Students are able to request copies of their documentation that we have on file. To request this, please email our office at ssd@uwec.edu 

Eligibility Letters that show what accommodations students have been approved for can be obtained from the or by contacting our office. 

Are you already registered with SSD and need to provide additional documentation or update diagnosis?

If you are looking to get additional accommodations because of a change in what was originally arranged with your Accommodations Coordinator (IE: new diagnosis, living arrangements, or injury). Documentation pertaining to the change can be submitted to our office. Please email this documentation to ssd@uwec.edu or connect with your assigned Accommodations Coordinator to discuss further.
